About Shlomit
Shlomit Liberty is a seasoned medical professional and Board Certified Independent Patient Advocate. Trained as a Medical Speech Language Pathologist, she has extensive experience managing complex medical conditions across settings. Shlomit’s medical training combined with her expertise as a Patient Advocate position her to quickly navigate hospital systems and other care facilities to ensure that patients receive timely and appropriate care.
Shlomit specializes in advocacy during inpatient admissions (acute hospitals, acute rehabilitation, and sub-acute rehabilitation), as well as post-discharge community re-integration. She works directly with patients in hospitals and medical centers across New York City, ensuring that their preferences, values, and wishes are respected while navigating their care. Whether it’s communicating with medical teams, facilitating family meetings, or assisting with discharge planning, Shlomit ensures that her clients’ needs are met with empathy and clarity.
Over the past decade, she has worked at top hospitals including San Jose Regional Hospital, Stanford University Hospital, and New York Presbyterian Weill Cornell Medical Center.
